That's the best I could find. I don't think the guitar only had 12 frets, it just had the top frets cropped off so he could put "NOT" on there.
As a side note, now you know why Scott Ian shaves his head
Anthrax's Nice Fucking Video ('87-'88) has him playing it. Yeah, he has bad hair..lol.

Thats crazy. I wonder if he still uses it?

I think all their old gear was destroyed in a fire in the early 90's, including that guitar unfortunately.
Scott retired the NOT guitar when it became a common 'trendy' word to use. He felt like it was being over-used.
