Wiring Help

hey i just threw a SD jazz and Jb into my s7420 but i'm having some trouble figuring out how to wire/solder the five way switch,

i've looked at the SD site but they only have a diagram for a super 5 way and a 5 way w/split

can anyone help me out ?
